  • What is Remote Patient Monitoring?

    Remote Patient Monitoring (RPM) is an automated process of collecting physiological data from a patient remotely. This data is used by the patient’s healthcare provider to help them better manage the patient’s chronic condition(s) thereby reducing the likelihood of unnecessary hospital admissions and improving the patient’s overall outcomes.

  • What are the benefits of Remote Patient Monitoring?

    Remote Patient Monitoring (RPM) offers many benefits to both provider and patient.  For healthcare providers, RPM enables them to monitor their patients continuously and remotely, giving them better insight into the patient’s current condition. Combined with your current Chronic Care Management (CCM) services and the physiologic data received from Remote Patient Monitoring, providers are given valuable insight to make decisions that can improve patient outcomes and intervene when needed.

     

    RPM helps patients experience a closer connection with their healthcare providers. Patient care is greatly enhanced as a result of the increased decision-making abilities based on regular biometric readings and system alerts regarding the patient's status.  In many cases, RPM will help patients avoid costly hospitalizations or trips to the emergency room. Additionally, patients minimize the risk of exposure to COVID-19 by avoiding non-essential visits.


  • What are the costs to implement a Remote Patient Monitoring Program?

    There are zero upfront costs to get started with our remote patient monitoring program. 


    We charge a flat rate per patient monthly for our services.  You will receive your first invoice after an initial 30-day cycle is completed for your patients.  The payment terms are 30 days which allows you plenty of time to file claims and get reimbursed before paying the invoice.


  • Will Medicare reimburse for Remote Patient Monitoring?

    Yes, Medicare (and Medicaid in most states) will pay for Remote Patient Monitoring. 


    Reimbursement for RPM services varies by each patient’s plan.  Some commercial carriers also provide reimbursement and we expect that number to grow in the future.


    The average reimbursement from Medicare for Remote Patient Monitoring services is below:


    • 99453 - Initial set-up and education of the patient (one-time fee) - $22
    • 99454 - Interactive devices supplied with data tracking system to track physiological measurements (monthly) - $72
    • 99457 - Initial 20 minutes of clinical time spent for remote care services (monthly) - $52
    • 99458 - Additional 20 minutes of clinical time spent for remote care services (monthly) - $42

  • What impact will this have on my staff?

    We know how busy medical practices can be. We do our very best to minimize the impact on you and your staff. The initial onboarding process will require a minimal amount of time from you and your practice manager or designated representative to assist us with preparations for onboarding. This includes things such as:


    • Identifying patients you wish to enroll in the program
    • Selecting devices for each patient based on their chronic condition
    • Providing us with patient information
    • Setting up the care plan and communication protocols with you and your staff for the program

    As part of the patient onboarding process, we handle all of the device logistics. We order and ship devices to go directly to the patient’s home.  Our clinical staff will train your patients on how to use the devices. We will monitor vitals and check in with patients who are not being compliant to encourage them to use the devices more often, citing the health benefits of doing so.


    Our clinical staff will teach patients how to use the devices and explain how the RPM program works for them. Care Specialists will monitor vitals and alerts and check in with patients to ensure compliance.  Maybe a mix of both.


  • What type of devices are sent to my patients?

    We will provide your patients with cellular-enabled, FDA-approved medical devices. These devices are incredibly easy for your patients to use. Your patient does not need internet service or a smartphone to use our devices. The patient will simply use the device and the data is submitted using encrypted cellular technology to our secure, HIPAA compliant database.  It’s that simple.  


    We believe strongly that cellular-enabled devices are the best choice for your patients due to their simplicity.  Patients are more likely to be compliant if the devices are easy to use.


    If desired, Bluetooth device options are available as well.


  • Can Access Telehealth integrate with my EHR?

    Access Telehealth offers integration options to ensure data will move in a timely and accurate manner across your IT system.  Key data elements including program eligibility, demographics, health information, HIE messages, alerts, and connected device data can be shared as needed.


    We leverage API's to offer one-way or bi-directional data integration with Electronic Health Record (EHR), Health Information Exchange (HIE), and other systems, such as EPIC, eClinicalworks, Practice Fusion, Cerner, and more using HL7/FHIR integration.

  • Can I purchase software and devices only?

    Absolutely. If you have the clinical staff to monitor your patients, you can simply pay us a monthly fee to use our cellular-enabled devices and monitoring software. If at any time, you wish to upgrade to our full service where we offer the monitoring, you can always do that.

  • What is the difference between Chronic Care Management and Remote Patient Monitoring?

    Remote Patient Monitoring (RPM) is an automated process of collecting physiological data from a patient remotely. This data is used by the patient’s healthcare provider to help them better manage the patient’s chronic condition(s) thereby reducing the likelihood of unnecessary hospital admissions and improving the patient’s overall outcomes.


    Chronic Care Management (CCM) includes non-face-to-face services provided to patients with two or more chronic health conditions that are expected to last for more than twelve months.  A care plan is established for the patient to help manage their chronic conditions, referrals to other providers, medication management, and an ongoing review of the patient’s health status.


    Combining Chronic Care Management services and the physiologic data received from Remote Patient Monitoring, providers are given valuable insight to make decisions that can improve patient outcomes and intervene when needed.


  • Who does the billing for Remote Patient Monitoring services?

    At the end of each billing cycle, we will provide you with a detailed report of time spent with each patient. This report includes CPT codes and dates that time increments were achieved.  Your billing department can easily take this report and use it to bill the health plan.
