Cellular and auto-pairing Bluetooth devices make it easy for patients to take readings. Access Telehealth provides device logistics services, including shipping, support and training on how to use the devices.
Automated Out-of-range and Compliance Alerts
Device readings are received by the platform 24/7 and processed against preset parameters to identify out-of-range values. Automated alerts are sent via text and/or email to designated providers or care teams when a physiologic reading is out-of-range. Remote Patient Monitoring is not intended to provide emergency care. The RPM program is for ongoing evaluation and management of patients with chronic conditions and triage is based on your organization's configured protocols.

Chronic Conditions
For several years now, CMS has focused on the expansion of Remote Patient Monitoring and Chronic Care Management programs to improve patient outcomes related to chronic disease. As a result of examining chronic conditions among Medicare beneficiaries, RPM guidelines have been established to better address the needs of these high risk patients and to improve resource utilization and reduce the burden on our healthcare system.
CMS has identified multiple chronic conditions that would benefit from remote connected care at home. CMS does not limit RPM to these conditions or patients over 65 years of age. RPM is based on medical need and is not necessarily limited to these conditions. Below, are some of the more common conditions where patients benefit from the programs and devices available:
